學生平安保險

主　旨：公告115-116學年度「弘光科技大學學生團體保險」之相關事宜。

說　明：

一、「115-116學年度學生團體保險」保險由「三商美邦人壽保險股份有限公司」承保，相關險投保內容，詳情參見「三商美邦人壽契約條款」。

二、保障對象：依據弘光科技大學「學生團體保險辦法」，具有本校學籍(含休學有繳學生團體保險費者)。

三、保險保障期間：自115年07月31日午夜12時起至117年07月31日午夜12時止(含應屆畢業生至當年8/31終止；上學期畢業者至當年2月月終之日午夜十二時終止)。

四、每學期保險費用：

五、理賠申請準備文件，詳情參見「115-116學年度學生團體保險理賠內容」，請填妥申請書及相關理賠文件後，請繳至本校衛生保健組(D棟107室)。

六、如無法親送或託人代繳者，亦可選擇郵寄（遺失或損毀風險自行負責），掛號郵寄至433台中市沙鹿區台灣大道六段1018號，衛生保健組收。

七、115學年度「三商美邦人壽」將提供以下服務事項：

(一)駐點服務：自115年8月起(寒暑假及例假日無服務)，

    每週二上午10:00-11:30於衛保組(D107)駐點，提供辦理團體保險理賠收件及保險諮詢事宜。

Subject: Announcement regarding relevant matters of the “Hungkuang University Student Group Insurance” for Academic Years 2026–2027 (the 115th and 116th Academic Years).

Description:

1. Insurance Provider: The Student Group Insurance for Academic Years 2026–2027 is underwritten by Mercuries Life Insurance Co., Ltd. For details on coverage and policies, please refer to the  Mercuries Life Insurance Contract Terms.
2. Eligible Beneficiaries: In accordance with the Hungkuang University Student Group Insurance Regulations, students who hold valid enrollment status at the university (including those on a leave of absence who have paid the student group insurance premium) are covered.
3. Insurance Coverage Period: From midnight on July 31, 2026, to midnight on July 31, 2028. (For current year graduates, the deadline is August 31 of that year; for those graduating in the previous semester, the deadline is midnight on the last day of February of that year).

5. Insurance Claim Submission Please refer to the 「Student Group Insurance Claim Content for Academic Years 2026–2027 」for the required application documents. Once you have completed the application form and gathered the required documents, please submit them to the Health Services Section (Room D107).
6. If you are unable to submit the documents in person or authorize someone else to do so on your behalf, you may mail them via registered mail. (Please note: The sender must assume all risks of loss or damage during transit).

7. On-Site Consultation Services (Starting August 2026)

• For the 2026 academic year, Mercuries Life Insurance will provide the following on-site services (excluding summer/winter vacations and public holidays):

• Services Provided: Collection of group insurance claim documents and insurance consultations.
